• Returning Patient Visit: $50

• Unlimited Care Plan (Individual): $150
  – Unlimited visits for 1 person over 3 months

• Family Unlimited Care Plan: $250
  – Unlimited visits for up to 3 household members over 3 months

All visits are self-pay. Superbills are available upon request for potential insurance reimbursement.
